The Adolescent Health Clinic was started to address health-care issues for teens that do not have a family doctor. It operates out of a youth centre in downtown Hamilton.

This clinic benefits teens while also helping family medicine residents get exposure to social accountability outside of our regular clinic setting.

Our clinic has a sports medicine doctor available to see our patients one day each month.

Sports medicine is a branch of medicine that deals with physical fitness and the treatment and prevention of injuries related to sports and exercise.

Our system navigators are here to connect you with the supports and resources that can help in all aspects of your life and health.

This can include concerns about:

• Income
• Housing issues
• Education and employment needs
• Social and/or recreational opportunities
• Access to food
• Access to medications
• Legal issues

We also advocate with you and on your behalf to ensure programs and systems are working the way we know they should. We want you to be getting what you need.

During your pregnancy our lactation consultant is here to help you learn about breastfeeding, to teach you about making milk for your baby and to teach you how to know that your baby is growing well. After your baby is born, you may want to see the lactation consultant for breastfeeding support or for further help with breastfeeding your baby.

This clinic is for patients who are taking anti-coagulation medication (e.g. warfarin, often known as Coumadin) and require point-of-care anti-coagulation monitoring.

The clinic is coordinated by our nurses and pharmacists to work with you to monitor lab and point-of-care results (e.g. blood samples that are collected and processed on-site), adjust medication, and educate you and your family members.

Our pharmacists work closely with you, your family doctor and other members of the health-care team to ensure your medications are the best choices for you.

Our pharmacist will meet with you one on one in the clinic or by phone. In this meeting our pharmacist will:

• Ask you questions to gather a complete medication history
• Review all the medications you are taking (prescriptions, over the counter and herbal supplements) to check for any problems and to improve effectiveness and safety
• Assist with chronic disease management
• Answer your questions about your medications

The McMaster Family Health Team provides a variety of mental health services such as groups and individual counselling sessions to their patients.

The Mental Health Team is made up of six social workers. The team aims to provide you with clear treatment strategies by encouraging you to set goals. These goals help manage and overcome difficult life situations and other challenging issues impacting your life.

The team uses therapy models such as cognitive behavioural therapy, solution focused therapy and mindfulness.

Referrals can be made to the Mental Health Team through your family doctor or allied health care providers.

Memory issues can impact every aspect of your daily life; from cooking and cleaning, to driving and using transportation, and even planning and organizing your day.

The Memory Clinic is made up of a multi-disciplinary team including doctors, nurses, occupational therapist, pharmacists and social workers. After an assessment with the team, our goal is provide you, your family and your family doctor with a comprehensive assessment of all memory concerns regardless of age.

We hold our memory clinics once a month. For more information, please speak to your family doctor.

With close to 20% of the population living with chronic pain, the McMaster Family Health Team offers various groups for pain patients; more information can be found on our groups page.

Pain Consultations

An assessment by a family doctor with a specialty in medical management of chronic pain can be completed in our clinic. Referrals to this specialist can be made by your family doctor or nurse practitioner.

McMaster Family Practice hosts a weekly procedures clinic for our patients.

The Procedure Clinic aims to offer improved access to routine, non-urgent procedures including skin biopsies, knee and shoulder injections, cyst removals, and toenail resections. Referrals to the clinic can be made through your family doctor.

Please note that certain procedures (cosmetic skin tag removals and certain other procedures) are not covered by OHIP. While we are happy to perform these procedures, there will be a fee charged to you. Ask your family doctor at the time of referral whether or not a procedure is covered by OHIP in order to avoid any misunderstandings at the time of your procedure.

Lastly, given the limited number of appointment times we have available each week in the clinic, we request a minimum of 24 hours’ notice for any cancellations or re-scheduling requests. No-show appointments to the procedure clinic will be charged at a rate of $50.

Our diabetes program provides assessment, education, treatment and support for people with type 2 diabetes and pre-diabetes.

The diabetes program team includes a dietitian, nurse practitioner and pharmacists.

We work closely with your family doctor and will meet with you to reiew your medications, blood sugars, meal planning, activity levels and potential diabetes complications. We are here to help answer your questions and concerns to increasing your confidence, motivation and skills to assist you to better manage your diabetes and live a healthy lifestyle.
